The patient’s existing Primary Care Physician remains an essential member of the Hospice Team, approving the patient’s Plan of Care and maintaining regular communication with the Hospice RN.

  • Primary Medical Care: The attending physician is typically the primary or main doctor responsible for a patient’s care. They are often the one who diagnoses medical conditions, prescribes treatments, and oversees the patient’s general health.

  • Coordination of Care: The attending physician serves as the central point of coordination for the patient’s healthcare team. They work in collaboration with other healthcare professionals, specialists, nurses, and allied health staff to ensure a comprehensive and cohesive approach to treatment.

  • Treatment Planning: The attending physician is responsible for developing and overseeing the
    patient’s treatment plan. This includes determining the appropriate course of action, prescribing medications, ordering diagnostic tests, and making referrals to specialists when needed.

  • Communication with Other Providers: Attending physicians communicate with other healthcare providers involved in the patient’s care to share information, discuss treatment options, and ensure that everyone is aligned in the approach to managing the patient’s health.

  • Monitoring and Adjusting Treatment: The attending physician regularly monitors the patient’s progress and adjusts the treatment plan as necessary. This may involve modifying medications, ordering additional tests, or making referrals to specialists for more specialized care.

  • Patient Advocacy: The attending physician acts as an advocate for the patient, ensuring that their medical needs and preferences are considered in the decision-making process. They help patients make informed choices about their care and treatment options.
