Registered Nurse

The Hospice RN functions as the Patient’s Case Manager, maintaining communication with the entire care team to guarantee comfort, enhance the quality of life, and make adjustments to the Plan of Care in response to evolving patient requirements.

  • Patient Assessment: Conducting thorough health assessments to gather information about a patient’s medical history, symptoms, and overall health status.

  • Care Planning and Coordination: Developing and implementing individualized care plans for patients, considering their unique needs, diagnoses, and treatment goals. Coordinating care among healthcare team members.

  • Medication Administration: Administering medications as prescribed by physicians, ensuring proper dosage, route, and timing. Monitoring for any adverse reactions and educating patients about their medications.

  • Patient Education: Providing education to patients and their families about health conditions, treatment options, medication management, and preventive care practices.

  • Wound Care: Assessing and managing wounds, including dressing changes, monitoring healing progress, and providing education on wound care practices.

  • Monitoring Vital Signs: Regularly measuring and monitoring vital signs such as blood pressure, heart rate, respiratory rate, and temperature to assess a patient’s overall health.

  • Administering Intravenous (IV) Therapies: Administering intravenous medications, fluids, and other therapies as prescribed by healthcare providers.

  • Assisting with Procedures: Assisting physicians and other healthcare providers during medical procedures and examinations.

  • Patient Advocacy: Advocating for patients’ rights and preferences, ensuring that their needs are communicated and addressed within the healthcare team.

  • Counseling and Emotional Support: Offering emotional support to patients and their families, especially during challenging health situations. Providing counseling on coping strategies and lifestyle changes.

  • Health Promotion and Preventive Care: Promoting healthy lifestyles and preventive care measures. Educating patients about the importance of vaccinations, screenings, and lifestyle modifications.

  • Documentation and Record-Keeping: Maintaining accurate and up-to-date medical records, documenting patient assessments, interventions, and outcomes.

  • Collaboration with Healthcare Team: Collaborating with other healthcare professionals, including physicians, nurse practitioners, and allied health professionals, to ensure comprehensive and coordinated patient care.

  • Home Health Care: Providing care to patients in their homes, including monitoring chronic conditions, assisting with activities of daily living, and educating patients and caregivers on self-care.

  • Participation in Quality Improvement: Participating in quality improvement initiatives to enhance patient care outcomes and improve healthcare processes.
